PHS Reminder:

This is a reminder that students entering into 9th and 12th grades need an updated physical and/or immunization record turned in prior to the beginning of school for the 2024-2025 school year.
Additionally, if your child has a medical condition that will require medication throughout the school year or your child plans on playing any high school sports, you will need to get the required paperwork completed by your physician and turned in.
You can find all the appropriate forms needed for your student on the district website under the parent tab in Health Services Department > Health Service Documents.

We are asking that physicals, immunizations, and all required paperwork be turned in on or before August 9, 2024 so our district nurses have enough time to review the records.
